Chronic Disease Care

As Family Physicians, we are trained to diagnose and treat a broad spectrum of chronic conditions, including (*but not limited to*) diabetes, obesity, hypertension, high cholesterol, heart disease, asthma, depression, migraines and COPD. We work with patients to optimize their health through appropriate testing, treatment and referrals to specialists. We help to coordinate care with specialists if needed and provide continuity and support for them and their families.  Routine chronic care visits with the primary clinician are scheduled at intervals that are determined by individual patient need and disease management guidelines.

To better coordinate care for our patients with chronic conditions, we are also certified as a Level 3 “Patient Centered Medical Home” (PCMH) through the Blueprint for Health initiative, and the National Committee for Quality Assurance. We take enormous pride in having achieved and maintained this highest level of certification.
